Medical Requirements for school year 2010 -2011
Sixth Grade Students must have a physical dated after August 31, 2009. The physical is due on the first day of school and the student will not be allowed to attend class until the requirement is met. The physical need to be on a state school physical form filled out by a doctor with the date and signature of the doctor on it.
Dental exams are not due until May 15, 2011 and must be dated after November 15, 2009. The dental exams are due for 6th Graders only!
